Cancellation fees

In the anamnestic questionnaire, you were informed to pay for the treatment and services provided to COOL LOOK s.r.o. in a proper and timely manner. (hereafter referred to as treatment) in accordance with the Price List and Cancellation Fees issued by COOL LOOK s.r.o.
The client (hereinafter referred to as the patient) is obliged to arrive at the agreed place of the provided treatment at the ordered time (hereafter referred to as the reservation).

 

IN THE EVENT OF A LATE CANCELED APPOINMENT BY THE PATIENT (i.e. FEWER THAN 24 HOURS BEFORE THE BEGINNING OF THE RESERVATION), OR MISSING OF THE PATIENT'S APPOINMENT OR CANCELLATION OF THE DATE BY THE DOCTOR / DENTAL HYGIENE FOR LATE ARIVAL the patient is obliged to pay the fee (hereinafter referred to as the cancellation fee) depending on the lenght of the planned schedule of the provided treatment, namely:
1,250 CZK / hour in the case of APPOINTMENT AT THE DENTIST and / or
900 CZK / hour in the case of DENTAL HYGIENE APPOINTMENT.

A LATE ARRIVAL OF 15 MINUTES OR MORE CAN RESULT IN CANCELLATION OF THE TREATMENT RESERVATION AND PAYMENT OF THE CANCELLATION FEE. Each case will be assessed by a doctor / dental hygienist individually.

IN THE CASE OF THREE LATE CANCELLED APPOINTMENTS BY THE PATIENT OR THREE MISSED APPOINTMENTS BY THE PATIENT OR THREE CANCELLATIONS OF BY A DOCTOR / DENTAL HYGIENIST FOR LATE ARRIVAL, THE PATIENT CAN BE EXCLUDED FROM THE DENTAL OFFICE.

IF THE PATIENT DOES NOT ARRIVE TO THE APPOINTMENT OF ONE TREATMENT OR CANCELS TWO OR MORE HOURS APPOINTMENT, THE PATIENT IS OBLIGATED TO PAY A DEPOSIT OF UP TO 100% ON THE TREATMENT BEFORE HE/SHE IS OFFERED A NEW APPOINTMENT.

 

Explanation:

All appointments are reserved for a specific patient. At the given time, it is not possible to treat anyone else. Thus, a missed or late arrival results not only in a waste of time for the dental practice.

The patient can cancel the appointment in one of the following ways: in person, by phone (in the form of an SMS to +420 725 961 588), or by e-mail to recepce@cool-look.cz. The receipt of the cancellation information must be confirmed by the office.

Information on late arrival and delayed start of the service provided:
Most treatment cannot be accelerated or shortened. Each one consists of different demands, and requires an individual approach. In order for the treatment to be performed with the utmost care to the best of our knowledge and belief, it is necessary for the patient to arrive for the treatment on time, or shortly in advance. Early arrival of the patient for treatment is also necessary so that patients who have appoinments afterwards subsequently do not have to wait.

WE ASK FOR YOUR UNDERSTANDING AND APOLOGIZE IF WE ARE LATE DUE TO THE DELAYED TREATMENT OF THE PATIENT BEFORE YOU. THE DOCTOR IS NOT RESPONSIBLE FOR AN UNSCHEDULED, OR OTHER HEALTH, COMPLICATIONS THAT MAY OCCUR DURING MEDICAL OR SURGICAL PROCEDURES.
